Error Workflow - Workitem

Hola, tengo un error en un WF, una persona tenía en su buzón del Business Workplace, un workitem, el cual se ha iniciado 2 veces y ha recibido un evento finalizador, 2 veces tambien, pero esto no tendría que ocurrir, alguna idea?

I have a problem, I don't understand that workflow executes 2 times the same workitem "Ejecucion iniciada" and "Recibido evento finalizador". Only, is possible execute 1 time and you can see the error in the screenshot.
